SUPPORT WORKER

You will be expected to perform a variety of tasks, according to the operation of the service, these include:
Assisting with shadowing new staff, writing or assisting with Care Plans and risk assessments, monitoring people’s physical and mental health, recording and reporting outcomes. General household duties and social inclusion on a daily basis.

You will receive appropriate training, including a Level 3 diploma in Health and Social Care specific to the client group you work with

The tasks you will be asked to undertake are:

DELIVERY OF PERSONAL CARE AND SUPPORT – Required as needed to ensure the safe running of the service

• Washing/bathing/showering a person’s body and hair

• Mouth care such as the cleaning of dentures and teeth

• Dressing

• Toileting which may include changing continence pads and dealing with catheter and stoma bags

• Helping them to eat and drink

• Helping them with their personal appearance

• Prompting, giving or administering their medication

• Meal preparation including cooking of meals and serving meals and drinks

• Assisting with toileting

• Monitoring their physical, emotional and mental health and reporting any concerns

• Assessing and monitoring behaviour, mental capacity and support needs in conjunction with outcome based care plans.

• Cope with challenging behaviour and the effects of the individual’s mental capacity

• Write up daily records and charts as required

• Assist in the development of Care Plans and assessments

• Assist the service user to live independently as possible

• Carry out domestic duties as required

• Assist Service Users in Social Inclusion including access to the community, day centres and outside activities

• Support Service Users to access Housing Related Support
